ABOUT THIS POSITION:
The Reintegration Mentor will be a role model and will reinforce the adolescent’s individual treatment plan as well as continued support for the adolescent and family after completion of the program. The family support aspect of the position is vital to assisting in creating and supporting a stable and secure home life for the adolescent to return to after the program (if applicable). Support could include, but is not limited to, assisting the family in acquiring basic needs such as housing, food, parental/familial mental health, and/or substance misuse treatment.
  • Assist Clinician to assess needs and service options for clients and their families
  • Assist Clinician with connecting youth to family facilitating meetings and visits as necessary
  • Assess appropriate services and linkages in the community
  • Assimilate and coordinate discharge of clients to evidence-based programs and other community services
  • Advocate for youth and their families
  • Attend court or providers meetings as needed
  • Play a significant role in the client’s treatment team
  • Work on flexible schedule, which includes week and evening hours
  • Share On-Call hours with other staff
  • Develop knowledge base of community resources across Connecticut
  • Maintain connection with youth and family throughout treatment and beyond
  • Perform aftercare for REGIONS clients

  • Focus on Client Needs: Anticipate, understand, and respond to the needs of internal and external clients to meet or exceed their expectations within program parameters
  • Ability to communicate, cooperate and serve people from all cultural identities with respect, professionalism, integrity and fairness in a manner that recognizes the individuality of each person
  • Foster Teamwork: Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance program effectiveness
  • Regular and consistent attendance
  • Performs other duties as assigned by supervisor

SCHEDULE AND RATE OF PAY:
  • 35 hours per week
    • Monday, Tuesday 12:00pm-8:00pm, Wednesday 9:00am-5:00pm, Thursday and Friday 12:00pm-8:00pm
  • Hours may fluctuate to allow for later coverage to meet needs of clients and families in the community that can’t be met in the morning. Reintegration Mentors may also need to flex their schedule depending on program needs.
    • Both would be indicated in advance and allow for planning and is at the discretion of the Program Director. 
  • Rate of pay is $58,000 per year

MINIMUM QUALIFICATIONS/REQUIREMENTS:
  • High School Diploma/GED or equivalent with lived experience, and experience, training, and a history of working with adolescents - Required
  • Valid driver’s license to utilize company provided vehicle – Required
  • Understanding unique issues and needs of adolescents
  • Skill in Advocacy, Crisis Intervention and ability to function independently
  • Flexible hours/weekends and evenings
